HOW TO JOIN AS A STUDENT

Steps to Enroll:

Once you have decided to enroll we will need three items to start your training:

(1) A filled out Student Application.

• Find the Adult Application HERE
• or the Application for a Minor HERE.

(2) The Student Waiver at this LINK must be read and signed.

(3) Payment for the first month of lessons and the enrollment package must be made.

• Checks are made out to “On Mission Martial Arts” or “OMMA.”
• Payment can also be made by Cash, Debit card, VISA or Mastercard to your instructor.
